Clarksville Primary Care Center, our satellite Healthcenter, is part of Community Memorial Healthcenter’s commitment to provide quality progressive health care for the residents of Clarksville and surrounding areas.
From newborns to grandparents, we provide medical care for the entire family.
Our Office:
Appointments are requested for all office visits. Our office is open Monday through Friday from 8:00 a.m. to 5:00 p.m. For an appointment, call (434) 374-2773.
Emergency Care:
If you have an emergency after office hours, call Community Memorial Healthcenter at (434) 447-3151 or report to the CMH Emergency Room.
About Our Medical Staff:
Dr. Christopher Imber’s specialty is Family Practice. He is Board Certified by the American Board of Family Practice. Dr. Imber received his Medical Degree from Temple University School of Medicine in Philadelphia, Pennsylvania in 1995. He comes to Clarksville from Gilbert, PA where he has practiced since 2005. Dr. Imber is a member of the American Academy of Family Physicians.
Holly Wilson, FNP is a Family Nurse Practitioner. She received her FNP Certification from University of Virginia, Charlottesville in 1992 and her Masters of Science in Nursing with a specialty focus on Community Health Nursing in 1991. Mrs. Wilson's clinical experience includes working as a nurse practitioner with Boydton Medical Center. As a Family Nurse Practitioner at Clarksville Primary Care Center, she functions in a collaborative professional relationship with Dr. Imber to provide accessible primary care that is patient centered and focused on health promotion and disease prevention.
Fees:
Payment for office visits is expected at the time of each visit. Most insurances set a fee for the service of which they pay a percentage. The remainder may be covered by secondary insurance but is your responsibility.
We can be Primary Care Providers for Blue Cross/Blue Shield and Medicaid HMO's.
Your Referral:
A referral is the highest compliment we can receive, and we welcome any friends or family members you refer to our office. We will provide the same quality care and personal attention you receive.
